附屬機構借調至本校任職人員請繼續使用原有識別證，無需再向學校申請新卡。

Fortheinitialapplication,pleasecheckthe “initialissue”box.It’sfreeofcharge,butarecent1or2-inchbustandhatlessphotomustbeuploaded. ※如因(發卡後7日內感應不良、資料有誤、調換單位)等原因申請重製者，請勾選重製。

(換發並要更換照片者，請同時上傳大頭照) (換發期間舊卡將註銷) Incaseanewcardisrequiredwithinsevendaysaftertheoriginalonehasbeenissuedbecauseofpoortrackingquality,wrongdata,unittransfer,etc.,pleasecheckthe “renewedissue”box.Theoldonewillbenullified simultaneously.Thephotoonthecardcouldbechangedifthenewphotoisuploadedaccordingly. ※如因(遺失、卡片損毀、卡片掉色、改名字等等)申請換發者，請勾選繳費重製，先需至勵學大樓1F自動繳費機完成繳費，費用250元，並上傳繳費證明。

Personneltemporarilytransferredfromaffiliatedhospitalspleasecontinuetousetheiroriginalidentificationcard.ThereisnoneedtoapplyforanewcardfromtheSchool.Thesameappliestoteacherswhoare attendingphysiciansandschooldirectors. ※兼任教師申請前，需先經簽呈核定。

Adjunctteachersmaysubmitapplicationsonlythroughapprovedofficialdocuments.  上傳檔案說明Informationforphotoupload ※大頭照上傳檔名請使用身分證字號(身分證英文首字大寫) PleaseuseyourIDcardNo.(withthefirstEnglishlettercapitalized)asthefilenameofthephototobeuploaded. ※繳費證明檔名請使用(職號+繳費證明，如1050111繳費證明) Pleaseuseyouremployeeno.and”paymentproof”asthefilenameofthepaymentprooftobeuploaded.   領卡說明Informationforcardpick-up ※製卡約需三個工作天(不含例假日)，本室將以mail通知領取。
